        Preserving Workers’ Records

    Each employing unit must preserve the following records for five years:

    For Each Pay Period:

    • Beginning and ending dates
    • Largest number of workers employed each calendar week of the period

    For Each Individual Employed During The Period:

    • Name and Social Security account number
    • Total hours worked each week, if less than full time
    • Money wages paid (special payments included)
    • Reasonable cash value paid in any medium other than cash
    • Date hired, rehired, or returned to work after temporary layoff and date and reasons job was lost

        Facsimile Reports

    EW must pre-approve any individual or organization producing facsimile forms before distributing them to employers, accountants, and others.

    These companies include:

    • Commercial printers
    • Business forms companies marketing facsimile forms
    • Tax software developers writing computer programs with computer-prepared forms
    • Companies batch processing quarterly returns for employers using computers

    Employers, accountants, and others using the forms, software, services, and products above must require vendors to show their DEW approval letter.

        Employer-Filed Claims

    Use Form UCB-113 to report a mass separation of 25 or more workers on or about the same time. Use this form when you have no work to offer individuals and no job attachment exists between you and the worker. File these forms when 25 or more workers are separated upon approval by your local unemployment insurance claims office. Tell your workers the company requests a claim be started. To avoid duplicate effort the worker should not report to the local office until told to do so. Once the local office receives Form UCB-113, it will schedule the claimant’s visit. Download this form or get it from your nearest claims office. Do not use Form UCB-113 for a regular vacation period or any short-term layoff.

Employers may file UCB-114 when a worker has performed no services because of lack of work or worked all available hours but less than his customary full-time hours and earned less than normal full-time wages because of lack of work during a claim week (seven-day period ending on Saturday). Such worker continues to have a "job attachment" relationship with the employer and earns less than his unemployment insurance weekly benefit amount. Report all earnings, including vacation pay and holiday pay, applicable to the week claimed.

    You can file using this method for only six weeks per benefit year. After six weeks, the worker must report to the workforce center if still unemployed. Do not file UCB-114 if the worker isn’t able or available for work. These workers (claimants) must report to the nearest workforce center so we can investigate the situation.

     

        SCWages

    Quarterly Wage Report
    SCDEW-covered employing units must file Employer Quarterly Contribution and Wage Reports each quarter using Form UCE-101/120 (UCE-101 is the bottom half of Form UCE-120).

    1st quarter due by April 30
    2nd quarter report due by July 31
    3rd quarter due by October 31
    4th quarter due by January 31 

    We file workers’ wage records by social security number, thus it’s essential you report the worker’s correct social security in the proper space on Form UCE-120. Reporting a worker’s wages under an incorrect social security number can make the worker ineligible for unemployment insurance benefits should he become unemployed through no fault of his own. (If an employee has failed to apply for a social security number, the employer must get one for him.)

    We mail report forms to all employers by the 20th of the last month of each quarter. If you fail to receive forms you still must file reports.

    Your contribution report must show the total wages paid during the preceding calendar quarter for covered employment, including employment in South Carolina and employment performed in another state under the Interstate Reciprocal Coverage Arrangement. You must also report employment outside the United States by US citizens, if any. (Quarterly forms packages come with instructions.)

     

        Correcting Wages Reported Errors

    To correct erroneous wages reported in previous quarters, submit a separate schedule showing the employee’s name and social security number, the correct wage amount, and the quarter the error occurred. Upon request, we’ll provide Form UCE-120-C, Statement to Correct Information.

     

        Reporting Wages of Multi-State Workers

    When an employee works in South Carolina and another state, one of four tests determines whether DEW law covers that employee. Note: Uniform provisions with other states avoid conflicts and overlapping coverage when an employee works in more than one state for an employer. The application of any test must consolidate the reporting of the employee’s wages in one state or that test doesn’t apply.

     

        Employer Quarterly Contribution and Wage Report

    Form UCE-120
    Employing units covered by DEW law must file the Employer Quarterly Contribution and Wage Report each quarter.

    Regardless of how many people an employing unit employs, it must file Form UCE - 151, Employer Status Report. We use the form’s data to determine the employing unit’s liability status. An employing unit not liable should request the commission to determine its liability status.

    If you acquire a business or a portion of an employer, notify the commission no later than 30 days from the quarter’s end during which the acquisition occurred.

    An employing unit giving employment in South Carolina, either on a permanent or temporary basis, must file a status report with DEW. Do not report wages paid for employment in South Carolina to another state without DEW’s specific approval.

        SCWages Program System—Requirements

    The Diskette/CD Wage Reporting System tracks year-to-date wages to calculate quarterly excess and taxable wages. It automatically calculates contributions and contingency assessments due based upon an account’s assigned tax rate. The system also generates a summary page useful in completing the UCE-101, Employer Contribution Report.

    We developed the Diskette/CD Wage Reporting System, SCWages so employers can report quarterly employee wages using a PC diskette or CD instead of listing them on paper. You are not required to use the system.

    The system requires a PC running one of the following operating systems:

    Windows 95™
    Windows 98™
    Windows 2000™
    Windows NT™ or Windows XP™.

    The system requires 20 MB of free disk space for installation. You will also need a 3.5” floppy disk drive capable of writing to a high-density (1.44/1.2 Mb) diskette or CD burning capabilities. You must print forms on a laser jet printer.

    Compute Quarterly Contributions—Compute quarterly contributions, contingency due, and wage totals for any quarter.

    Prepare Filing Diskette/CD(s)—Prepare a filing diskette for a single employer or multiple employers for any quarter.

    Printing Capabilities—Available printing options:

    Employee List—Print an employee list, including quarterly and year to date total wages.

    Contribution and Wage Report—Print a contribution and wage report using blank forms from the DEW.

    Computed Quarterly Contributions Report—(For bubble jet, ink jet, impact or dot matrix printers) Print a report of total, excess, and taxable wages and contributions and contingency to be transferred to the DEW’s form mailed each quarter.

    Add, Update, Delete Employer Data—A simple way to enter additional employer records.

    Other Options/Features
    Find—Find employers by account number or name and employees by Social Security number, last name, or first name.

    Print Options (Prepare Filing Diskette/CD(s))—Print a hard copy of employees reported for each employer, employers reported on the diskette and/or the contribution report. Print multiple copies of “reports.”

    Sorting (Ascending/Descending)—Sort employees by Social Security number, last name, first name, and quarterly wages. In the Prepare Filing Diskette/CD process, users can prepare a hard copy of the employees reported via diskette/CD sorted by social security number, last name, or order in which the data was entered.

    Set Taxable Wage Base—In the event the taxable wage base increases or decreases, this option lets users set the new wage base for all employers in the database. Currently, the taxable wage base is $7,000 for each employee; setting this option is not required.

        Why Can’t I Have A Blank Form?

    We prefer you use our forms, though we accept facsimiles of the Employer Quarterly Contribution and Wage Report (Form UCE-101/120) only if produced to our exact specifications—4 ¼ inches by 8 ½ inches. Form UCE-101/120 (Available for viewing only.) If you can’t produce an exact facsimile, use our pre-printed form. For more details, see Why Can’t I Have A Blank Form?

    Details On Dual Forms UCE-101/120
    Forms UCE-101 and UCE-120 are printed on the same page; however, a perforation separates the forms. If you can’t perforate your facsimile, submit the forms on separate pages.

     

        Form Production Details

    The print color for Form UCE-101’s hand constraint area is Pantone 444 U at 10 percent density. Print for the remainder of the form is Pantone 444 U at 100 percent density. The vertical lines’ width between the hand constraint boxes is 20 thousandths of an inch and the vertical line’s width between the dollars and cents’ boxes is 30 thousandths of an inch.

    If the data in the hand-constraint area is typed, the print should be right justified. The hand-constraint boxes will not be necessary if the data is typed. Please note that items 2 – 5 are underlined; items 6 and 7 must not be underlined. Commas and dollar signs should not be printed in the data fields.
