        Tax Forms

    Employer Request for Benefit Address (Form UCE-101 BA) | Designate an address for all benefit documents.
    Employer Report of Change (Form UCE-101-S) | Report change of name, address, phone, etc.
    Statement to Correct Information (Form UCE-120C) | Correct information submitted on your Quarterly Contribution and Wage Report.
    Statement to Correct Information, Continuation Sheet (Form UCE-120C-A) | Continuation sheets to correct reporting of individual wages and totals.
    Employer Quarterly Continuation Sheet (Form UCE-120A) | Report wages for additional employees beyond UCE-120's limits.
    Employer Status Report (Form UCE-151) | Determine if you are liable for UI tax contributions
    Election to Become an Employer (Form UCE-154) | Elect to become an employer not otherwise liable under the DEW Law.
    Election to Become Reimbursable (Form UCE-155) | Elect payment options for benefits paid quarterly.
    School-To-Work Exempted Employment Certification (Form STW-UI-1) | Educational institution certifies to employer that a student is participating in a School-To-Work Program.

     

        Employer Claims Forms

    Offer of Work(UCB-261) | Report an individual who has turned down a suitable offer of work.
    Mass Separation Report -- Total Unemployment (UCB-113) | File for UI benefits for employees when laying-off 25 or more individuals.

     

        Appeal Forms

    Request for Appeal (APP-100) | File an appeal regarding UI benefits or tax.

     

        Facsimile Reports

    DEW must pre-approve any individual or organization producing facsimile forms before distributing them to employers, accountants, and others.

    These companies include:

    • Commercial printers
    • Business forms companies marketing facsimile forms
    • Tax software developers writing computer programs with computer-prepared forms
    • Companies batch processing quarterly returns for employers using computers

    Employers, accountants, and others using the forms, software, services, and products above must require vendors to show their DEW approval letter.
