Paper Filing—Facsimile Report

Why Can’t I Have A Blank Form?

We prefer that employers use the forms provided by this Agency, however, the SCESC will accept a facsimile of the Employer Quarterly Contribution and Wage Report (Form UCE-101/120) produced to our exact specifications. Test documents of facsimile forms must be submitted to this Agency for approval. A sample copy of Form UCE-101/120 is available here for viewing only. A test should include at least two reports, one with 10 or fewer employees and one with 20 or more. The forms should be printed on perforated paper or correctly sized. Contact information should be included and "TEST" should be written on the reports. The mailing address (include on inside of envelope also) is W. The Employer Contribution Report, Form UCE-101 (bottom portion of Form UCE-120), must be an exact facsimile of our form. If you are unable to produce an exact facsimile, the information should be transferred to our pre-printed form. The size of the form must be 4 ¼ inches by 8 ½ inches.

Forms UCE-101 and UCE-120 are printed on the same page; however, since the forms are separated when received by our Agency, there must be a perforation between the forms. If a perforation cannot be provided, the forms must be submitted on separate pages.

The print color for the hand constraint area of Form UCE-101 is Pantone 444 U at 10% density. Print for the remainder of Form UCE-101 is Pantone 444 U at 100% density. The width of the vertical lines between the hand constraint boxes is 20 thousandths of an inch and the width of the vertical line between the dollars and cents’ boxes is 30 thousandths of an inch.

If the data in the hand constraint area is typed, it is preferred that the print be right-justified. The hand constraint boxes will not be necessary if the data is typed. Please note that items 2 – 5 are underlined; items 6 and 7 must not be underlined. Commas and dollar signs should not be printed in the data fields.
